I worked really hard and recently lost 50 pounds which is a huge self esteem boost. However, I haven’t had dental insurance for a few years and have a difficult time making it paycheck to paycheck therefore I can’t afford to get my teeth fixed. I am only 24 years old and my front teeth are rotting out which makes me very self conscious and I don’t smile often because I am ashamed of showing my teeth. I plan on getting married April 2011 and it would be a dream come true to have my teeth fixed so my wedding day can be what I have always dreamed of.


I am nominating my mom because she deserves this makeover. For years now she hasn't taken any family pictures or portraits because she is embarrassed to smile. She is always giving and helping others and I would like to see someone help her for a change. She has raised 4 kids (2 that are still living at home) and cannot afford dental insurance. It upsets her a lot when people make fun of West Virginians for their smiles, but not everyone can afford a dentist. She is back in college trying to get a good career to finish raising my 2 other sisters who are still school age and live with her. She is afraid of rejection though when she finishes school and goes to job interviews. She also has never had a hairstyle or color she has been truly happy with and wants a nice look for her future career and she and start getting family portraits before my 3 year old sister gets much older. Every time she does try to any dental or health work done for herself,she ends up helping others and her kids instead, putting herself last. I can look back on family photos and see my mom smiling and I would like my 2 young sisters and my kids (her grandkids) to see a happy smiling mom and grandma. I want her to feel good for getting her college degree and to be able to go on job interviews and feel good about herself and her appearance for once in her life. And most of all I want her to have one less health problem to worry about with constant infections in her teeth. I just want her to be happy and this is the only thing holding her back. She needs a big smile to match her big heart!

I think the picture speakes for itself. I been a hair dresser for 36 years and have been so busy making everyone else beautiful I have no time for myself. I couldn't afford braces as a kid. please pick me.


I currently work at a local child care center. I try my best to keep a smile on my face for the kids, but it is hard. When I was about 15, I was told that I had very bad acid reflux. It got so bad I was put on medicine until it got better. But in that time, the acid ate away at my teeth. The dentist told me that at the time, there was nothing I could do until I got the acid reflux was under control. By the time it was, there was already too much damaged. It is had for me to do alot of the many things that I like to do. I volenteer at a local elementary school and some of those children can be very harsh. I have also had many people around me not want to get to know me because of my teeth. If I could win this makeover, I could better myself in many ways. I would feel better about talking to the people around me and it would also help my physical health. I could go through the day not wondering if I will have a tooth ache or if any more will fall out. I would do it on my own, but as of now I am newly married and tring to make it on 2 wages that are alittle about minimum wage. It is very hard as it is and I tend to push my self to the side for the bills that we already have. Winning this would mean the world to me. Thank you.

I am a proud mother of 3 beautiful girls. I have needed dental work for a while now but money is so short in this house. It seems like when we do have a little money the girls need something so we always put them first. I want my girls to be proud of me and like the way their mommy looks. They always point out to me that I need to go to the dentist and ask me why I dont go and I tell them that mommy will get her teeth fixed. I dont want to feel ashamed of smiling or talking to people because I am afraid of what they think of me. If you could help me I would be so forever in your debt.

CONTEST RULES for WVAQ and Wilson Martino Dental Associates Present

Extreme Makeover Contest
 
  1. By entering The Extreme Makeover Contest ("Contest"), sponsored by (the "Sponsors") the entrant acknowledges and agrees to abide by these Contest rules.
  2. No purchase necessary to enter or to win. Making a purchase will not increase your chances of winning.
  3. To be eligible, contestants must be U.S. citizens, WVAQ listening area residents and eighteen (18) of age or older.
  4. Void where prohibited. All federal, state, and local laws apply.
  5. Sponsors reserve the right to change Contest rules at any time, for any reason, without notice, in their sole discretion.
  6. Contest entry period begins Friday, January 1, 2010 and ends Friday, February 5, 2010. Limit one entry per person for the duration of the Contest.
  7. Entries will be taken exclusively online at www.WVAQ.com by listeners either nominating someone or entering themselves. A picture must be submitted of contestant smiling. The top five (5) finalists will be posted on Monday, February 8, 2010 and voting for the Grand prize winner online will begin. Voting will close on Friday, February 12 at 5:00P.M with the Grand prize winner announced on Monday February 15, 2010 at 7:40A.M.
  8. The Grand Prize:
    The Grand Prize consists of all of the following:
    1. 10,000 dollars in cosmetic dental surgery from Wilson Martino Dental Association 
    2. A dermatology makeover from Mountain State Dermatology
    3. Hair styling and facial makeover from Waterfront Spa & Salon
    4. One (1) year fitness club membership to HealthWorks
    5. Photographs of the before and after taken by All Pro Photography
  9. Grand Prize Winner must be prepared to disclose personal information regarding their medical history, psychological history, criminal history, employment history, and other similar information that has a bearing, in the professional judgment of the professional staff at Wilson Martino Dental Associates, Mountain State Dermatology, Waterfront Spa and Salon, HealthWorks and All Pro Photography. Failure of Grand Prize Winner to meet with, cooperate with, or make complete, accurate, and truthful disclosures to the staff of Wilson Martino Dental Associates, Mountain State Dermatology, Waterfront Spa and Salon, HealthWorks and All Pro Photography, may disqualify the Grand Prize Winner from further consideration.
As a condition of being selected as a contestant, entrants
    1. Authorize Sponsors and their designees to investigate, access, and collect information about the entrant, about any of the statements made in any entry materials, any supporting documents, or any other written or oral statements made in connection therewith;
    2. Authorize Sponsors and their respective designees to secure information about their experiences from current and former employers, associates, friends, family members, educational institutions, government agencies, credit reporting agencies, and any references provided by entrants, and entrants authorize such parties to provide such information to Sponsors and their designees;
    3. Agree to execute any authorizations, consents, and releases that Sponsors may request in connection with any background investigation activities;
    4. Unconditionally and irrevocably release and forever discharge all parties and persons from any and all liability arising out of or in connection with any background investigation;
    5. Agree to undergo to the extent permitted by law and at the sole discretion of the Judges any physical and psychological examinations requested by Judges in connection with possible selection as a Grand Prize Winner and agree that any such examination would be conducted by personnel of Judges' choosing and that any such examination does not create a confidential relationship between the entrant and examining personnel (or, to the extent necessary, waive any privilege that might otherwise attach to such a relationship), and, accordingly, consent to such personnel communicating to Judges or Sponsors any diagnoses, prognoses, medical information and/or opinions regarding entrant.
  1. Odds of being selected as the Grand Prize Winner depend on the number and of entries received, which shall be determined in the sole discretion of the Judges.
  2. Grand Prize Winner will be notified by phone, US Mail or in person. Grand Prize Winner must respond within twenty-four (24) hours of receipt of notification of their Grand Prize Winning selection. In the event that a notification of Grand Prize Winner selection is returned to the Sponsors as undeliverable or that the Grand Prize Winner fails to respond to notification within 24 hours, Sponsors reserve the right to name an alternate Semi-Finalist.
  3. The Grand Prize Winner(s), in consideration of winning the Grand Prize, grants Sponsors the rights to, and agrees and consents to use and reuse by the Sponsors and any of their respective licensees, assignees, parents, subsidiaries, and affiliated entities and each of their respective employees, agents, representatives, officers and directors of his or her name, photos, image, voice, actions, appearance, biographical material, information contained in entry materials, and likeness, for purposes of publicity, advertising, trade and promotion, without further compensation, unless prohibited by law. The Grand Prize Winner(s) acknowledge that Sponsors may video and audio record the Grand Prize Winner(s) before, during, and after the performance of the elective cosmetic procedures, and that portions (selected in Sponsors' sole discretion) may be aired on WVAQ. The Grand Prize Winner(s) specifically consent to any such audio recording and its use and reuse by and on WVAQ.
  4. Grand Prize Winner(s) must be available for several days (in the aggregate) for the performance of the makeover, Actual dates and times may vary. The makeover will be performed over the course of approximately 10 weeks. Actual dates and times may vary.
  5. Grand Prize Winner(s) will be selected in the sole professional discretion of the professional staff at, all other Sponsors, and each of their respective parent companies, agents, affiliates, subsidiaries, and employees assume no role or responsibility in the selection of the Grand Prize Winner(s) nor in the performance or results of the makeover. PARTICULAR RESULTS ARE NOT GUARANTEED.
  6. Entrants understand that the Grand Prize involves elective cosmetic surgery and that elective cosmetic surgery poses certain risks to patients. Entrants acknowledge that before participating in the Contest they should consult with their own physician at their own expense regarding their physical and emotional health qualifications for elective cosmetic surgery. By participating, entrants represent that they know of no reason, following consultation with their own physician, why they should not participate in the Contest.
  7. Entrants and the Grand Prize Winner(s) must release, discharge, relinquish and hold harmless the Sponsors and any of their respective licensees, assignees, parents, subsidiaries, and affiliated entities and each of their respective employees, agents, representatives, officers and directors ("Releasees") from any and all liability for medical care assistance, treatment, or services provided at any time, whether such treatment or services are provided by health care professionals, paramedics, or any other person.
  8. Grand Prize Winner must execute all waivers and release agreements required by any or all of the Sponsors. Refusal, inability, or incapacity to execute any waiver or release disqualifies the refusing, disabled, or incapacitated Grand Prize Winner.
  9. Releasees are not responsible for lost, late, or misdirected entries, or for incorrect, inaccurate or incomplete entry information whether caused by a contestant, equipment, or technical malfunction or for any human error, technical error or malfunction. Releasees are not responsible for failure of any electronic equipment, computer transmission, or network or phone line connections.
  10. Grand Prize Winner(s) must accept all components of the Grand Prize. Failure to do so automatically disqualifies the Grand Prize Winner, and an alternate may be selected from the remaining entrants.
  11. Prizes cannot be exchanged for cash. Sponsors reserve the right to substitute a prize of comparable or greater value in the event the offered prize is unavailable.
  12. Transfer of prizes is strictly prohibited. Receipt and use of all prizes shall be conditioned upon compliance with any applicable federal, state and local laws.
  13. Any federal, state or local tax or other fees or charges associated with the receipt or use of any prize is solely the responsibility of the Grand Prize Winner(s). To meet Federal Tax Regulations, Grand Prize Winner(s) may be required to present his or her Social Security Number to Sponsors upon request. Prizes may be considered income.
  14. Grand Prize Winner may be required to submit valid identification to the Sponsors and to execute an affidavit of eligibility and a liability release and a publicity release, where legally permissible, prior to being awarded the Grand Prize or any component thereof. If a Grand Prize Winner fails to submit any such information, as requested, such Grand Prize Winner will be disqualified and a new one may be selected from the alternates.
  15. Releasees assume no responsibility or liability for damages, losses or injuries resulting from use of or acceptance of any component of the Grand Prize. In consideration for winning the Grand Prize, Grand Prize Winner(s) agree to release all Releasees from any and all liability for claims, actions or proceedings for injuries (INCLUDING PERSONAL INJURY OR DEATH) or damages sustained in connection with the receipt, ownership or use of any component of the Grand Prize, or while traveling to, preparing for or participating in any Grand Prize or Contest related activity, except as otherwise limited by applicable law.
  16. The Sponsors reserve the right, in their sole and absolute discretion, to interpret compliance with the letter and intent of these rules, and to disqualify any entrant deemed to have violated these rules or to have filled out any application or submitted any entry materials incorrectly, incompletely, or untruthfully at any time prior to the distribution of the Grand Prize or any of its components.
  17. Immediate family members and employees of the Sponsors, and each of their respective parent companies, agents, affiliates, subsidiaries, suppliers, prize suppliers for the contest and their immediate families (spouse, and parents, children, and siblings, and their respective spouses) and individuals living in the households of the above and advertising agencies, are not eligible to participate in this Contest.
  18. Wilson Martino Dental Associates:
    1. Appointment Times: The Doctor Assigned to the case and the location of the work to be done is solely at Wilson Martino Dental Associates discretion. Wilson Martino Dental Associates will try to accommodate the patient’s request but reserve the right to assign appointment times, Dr’s and location of the work to be done.
    2. Wilson Martino Dental Associates will allow a six (6) month time period from the date the winner is announced to have work completed.
    3. The amount awarded is not transferable.
    4. The work that needs done is at the sole discretion of the Dentist assigned to the case from Wilson Martino Dental Associates. All work to be performed will be for the patient’s good and long-term oral health. If it is determined that an amount over 10,000.00 is not needed then the remainder of the award will be forfeited. If it is determined that an amount over 10,000.00 is recommended then it will be the sole responsibility of the patient to pay for additional work under Wilson Martino Dental Associates normal payment arrangement policies.
    5. The extreme makeover procedures include all procedures currently offered by Wilson Martino Dental Associates at Wilson Martino Dental Associates normal fee schedule excluding: Implants, and oral surgery excluding extractions, and any procedure detrimental to the patient’s long term health.
  19. Releasees, advertising and promotion agencies, judging agencies and their immediate assigns are not responsible for:
    1. Problems associated with radio transmission, satellite transmission, or phone lines that are beyond the control of Sponsors. This includes, but is not limited to: Telephone disconnects, operator interference, cellular interference, technical or atmospheric conditions that disrupt the completion of any phone call, and technical difficulties which may prohibit the transmission of their radio signals to all or limited geographic areas during the playing of the Contest;
    2. Failures of computer hardware, software, or similar systems.
    3. Typographical errors in any on-air presentation and/or printed materials relating to the Contest;
    4. Cancellations, postponements, or delays of any kind, including but not limited those relating to the Contest, the Grand Prize, or any components of the Grand Prize;
    5. Breaking news, a natural disaster, an act of God, war, or terrorism that make it inadvisable, illegal, impossible, or impractical, in the Sponsors' sole discretion to conduct the Contest or any part thereof or otherwise perform under these Contest rules.
Any attempt by an entrant to tamper with the proper administration of the Contest will disqualify the entrant and may result in the discontinuation of the Contest. All decisions are made by the Sponsors in their sole discretion, and are final.

  2. These Contest rules shall be interpreted in accordance with West Virginia law, without regard to its conflict of laws rules. Any suit, action, or proceeding arising out of or connected with this Contest or any prize awarded under these rules, shall be brought in Morgantown, West Virginia and entrants, Semi-Finalists, and Grand Prize Winners specifically consent to jurisdiction and forum in Morgantown, West Virginia
